    Checklist instructions

    • When first creating this checklist: Before publishing this checklist, make sure a checklist does not already exist in your Test Site project.
    • When first creating this checklist: Make sure to publish this checklist to your Test Site project by scrolling down near the bottom of the checklist and selecting "change" next to the Care Coordination and Integrated Case Management and type in the name of your Test Site.
    • Updating the checklist: To update this checklist as you progress through the steps, click on Edit along the right side. When finished updating, scroll down to the bottom and click on Publish

    • Initiation Activities: Work through the activities listed below
    • Update this checklist as you complete each step. Whenever you start a new Phase checklist, make sure to copy and paste the Phase Checklist to the corresponding column in the Monthly Operational Review - November 2015 for tracking purposes

    1) Identify VAMC Sponsor / Executive Leadership

    **Edit this checklist to track each step by clicking on Edit along the right side of this document and adding a emoji or typing "Yes" in the Complete column upon completion**

     

     

    • Upload the signed copy to your Test Site Project
    • Add your Test Site name to the title of the file
    • Before you publish, assign it to the "initiation phase"The specified item was not found.
    • Update this checklist by clicking Edit along the right
    • Copy and paste the URL to your copy of the ICC-ICM MOU file in the Notes column
    • Update the CC & ICM Implementation Dashboard by going to the dashboard document, clicking Edit along the right side and adding the URL for this checklist to the Initiation Phase column for your Test site

    2) Identify RN and SW Co-Champions

     

     

    • Upload your copy of the Team Planning Roster with the Sponsors and Leaders section completed to your Test Site Project
    • Add your Test Site name to the title of the file
    • Before you publish, assign it to the "initiation phase" The specified item was not found.

     

     

    • Update this checklist by clicking Edit along the right
    • Copy and paste the URL to your copy of the Team Planning Roster file in the Notes column

    3) Assemble key stakeholders

    • Download your copy of the Team Planning Roster you submitted above
    • Complete the Participants Section

     

     

    • Open your VA Pulse version of the Team Planning Roster and click Edit along the right side. Upload your updated file OVER the old file

    **You do NOT need to upload a brand new file / delete the old one. You can overwrite the old file which will still be stored as an archived version for historical purposes (and reduce confusion with multiple files)**

    • Add your Test Site name to the title of the file if necessary
    • Update this checklist by clicking Edit along the right
    • Copy and Paste the URL to your copy of the Team Planning Roster file (it should be the exact same as above)
    • Add (Complete) to the title of this checklist
    • Scroll down in this checklist (while in Edit mode) and delete the incomplete tag. Add a tag called complete
    • Hit Publish to save your changes
