You local Jive development environment probably isn't market verified, which is why the App isn't showing up.
WRT to the sandbox, because you are using the Jive SDK template, you will need to at-least click the Configure Now button in order to activate the add-on .. .when the configuration URL is present in the meta.json, this triggers this behavior in the add-on.
What happens after you click through the Add-On Configure Now experience and then click Save and Activate?
After I setup "Configure Now" and click "Save and Activate", it will go back to the installed add-on page. see below:
I checked again, the App function works well even through the App can't recognized in upload page.
