This is odd. Can you share a snippet from your app.xml definition?

and from personal experience, your action paths appear to be correct.
Can you confirm the URL in the browser when you get this error? Does the ending token match the appPath of your application definition.json?
Any other contextual information you can provide would help us to figure out the issue.
It is odd, especially when it was working perfectly weeks ago.

My application does not have definition.json ,I used Jive web app template to create my project. In addition other launchers are working without any issue, jive/actions/rte, jive/actions/content/*, jive/tabs/actions

One thing I noticed, that I would recommend ... is a bit of cleanup of your actions section into a single CDATA, rather than multiple.
One suggestion I would do is to define your app appPath, similar to this:
This should change the resolvable path in the UI and hopefully correct the appPath rendered for your app.

I have also tried to install the sample todo app to see if the tab in places would work:
But I'm getting the same unauthorized error. Are other people having this issue? What else can I do? I really need to get this to work.
Varand, can you confirm that your add-on is still deployed? I am not seeing it in the list, and I believe the group you are using to test this on:
Doesnt show the tab as well?
Were you using DevConsole to deploy this app, or were you using an Add-On?
