Just found out, below discussion for apps:
The application name, i.e. what's in the app.xml is the 1st order set. Note, if there are any leading spaces / white space in the name, this counts and it will shoot the action to the top of the list.
Next level is the action name, i.e. what is being displayed as the link name

Yes, you are right because of the unintended spacing of the name. It went on top!!
But, I want to know the path you defined in app.xml?

I have added link component in plugin.xml of my plugin as has been mentioned here:
Since I needed to insert link in actions tab inside content, I used "Document Page Actions" section.
This is what makes link inserted in above manner.
Also my issue is for plugin and I can't say how it works for app. But below format should be valid for all.
